Home and Community Care
Most people want to live in their own home, but some people can find this difficult without support. Help can be provided by the Home and Community Care (HACC) program and other support services.
HACC can provide basic support services so people can continue to live in their community. These services provide eligible people with help at home, support in getting out into the community, and a break for carers.
HACC services are provided in your home or local community and may be provided by a HACC agency or other community organisations. HACC is jointly funded by the Australian and ACT Goverments.
Who is eligible for HACC services?
To be eligible you need to be:
- living in Tuggeranong or Weston Creek
- have a health issue that makes everyday tasks such as showering, dressing or doing housework difficult
- unable to access public transport
- be at risk of having to move to residential care
- be caring for a person who needs ongoing support

Assessment for a HACC service.
To assess your needs, one of our staff will talk to you about how you are coping with your daily living tasks. We will fill out a form called a Client Information and Referral Record. This helps us identify what type of service will assist you to remain living in your own home.
