What is HACC?
Home and Community Care is for people who need support services to assist them to live in their own home. For example, HACC can help with everyday tasks such as:
- housework
- shopping
- showering
- getting dressed
Other services include:
- Transport services to medical appointments and social outings
- Social activities at the Centre4Seniors.
Who is eligible for HACC?
You need to be:
- living in Tuggeranong or Weston Creek
- have a health issue that makes everyday tasks such as showering, dressing or doing housework difficult
- be at risk of having to move to residential care, or
- be caring for a person who needs ongoing support
To be eligible for transport assistance, you must be unable to access public transport.
A small fee may apply for some services.

What happens next?
One of our friendly staff will talk to you about how you are coping with your daily living tasks. We will fill out a form called a Client Information and Referral Record. This helps us identify what type of service will assist you to remain living in your own home.
What can you expect from HACC?
- Your own caseworker will visit you in your own home and work with you to develop a care plan that reflects the services that you need to support you at home.
- We will also arrange your care and phone you often to ensure that the services are meeting your needs.
- Our Client Service program provides a package of support service to assist you with short-term care until we can support you with ongoing services from a generic Home and Community Care service.
HACC Community Transport
The transport program provides a vehicle and driver to people who are unable to use public transport.
- If you have no other way of getting to medical or social appointments, our experienced staff and volunteers can assist you from door to door.
- Qualified drivers from our team can be booked from between 7.00 am and 7.00 pm Monday to Friday. Limited trips are available on weekends.

Shopping
A shopping helper can drive you to a Shopping Centre and assist you to do your grocery shopping, banking or other retail tasks. They can also assist you with carrying some shopping bags.
Friendship visits
Volunteers come to your home on a regular basis to spend time with you talking or reading over a friendly cuppa.
Centre4Seniors
To experience great programs, gain support or make new friends, the Centre4Seniors , located in Weston Creek, is for you.
- enjoy quality recreational programs and diverse activities
- A two-course hot lunch is served Wednesday – Friday
- We charge a nominal fee for activity materials, lunches and morning and afternoon teas
- There's no need to become a member or join the Centre

Men’s Group Lanyon
This is a Men’s only social group and a chance to enjoy some mateship. Participants work on their own wood working projects or build toys for local charities. Tools and facilities are provided at the historic Lanyon Homestead. Support and guidance is offered by a team of volunteers. Generally, the group meets on a Thursday morning from 10.00 am – 1.00 pm. There is a voluntary contribution of $6.00 to cover materials. Morning tea and a BBQ lunch is also provided. Transport can be arranged.
Connections@Cooleman
Connections@Cooleman is an activities based leisure and living skills program designed for adults with disabilities between the ages of 25 and 40 years living in the Tuggeranong or Weston Creek regions.
