Home and Community Care (HACC)

Fun 4 Youth

Connections@Cooleman
Connections@Cooleman is an activities based leisure and living skills program designed for adults with disabilities between the ages of 25 and 40 years living in the Tuggeranong or Weston Creek regions.

Transport Services
The transport program provides a vehicle and driver to people who are unable to use public transport. If you have no other way of getting to social or medical appointments, our experienced staff and volunteers can assist you from door to door. Click Here to find out more.
