About the Team  

Communities@Work's Lifestyle Programs (Seniors, Disability, Transport and Support Coordination Services) deliver community support to seniors and people living with a disability in an environment characterised by trust and respect. About the Role 

Communities@Work adopts a client-centred approach to the development and delivery of its services in response to identified client needs.   

We deliver support services to people with a disability and/or mental health issues and seniors to enhance their independence, social inclusion and well-being that is characterised by trust and respect. 

Under the day-to-day direction of the Seniors Coordinator, the Wellness Activities Facilitator requires an innovative, proactive and enthusiastic approach to service delivery to achieve program goals and aspirations. 

The Wellness Activities Facilitator is responsible for assisting with designing and implementing a range of diverse, innovative and stimulating social activities where wellness, capacity building and restorative approaches are central.  

The Wellness Activities Facilitator will deliver workshops or activities that could include, but are not be limited to, art and craft, nutrition and cooking, life skills, fitness, technology or academic topics of interest to aged persons. 

 

Duties 

  • Plan and deliver activities within a Wellness and Reablement framework 

  • Contribute to the delivery of services using a best practice approach that is consistent with legislative requirements, the Aged Care Quality Standards and National Standards for Disability Services.    

  • Undertake risk assessments and implement risk management plans for all activities delivered in the program 

  • Facilitate agreed group activities/workshops that could include, but are not be limited to, art and craft, nutrition and cooking, life skills, fitness, technology or academic topics of interest to aged persons 

  • Assist in the preparation of the centre for the planned activities and ensure the centre environment is clean and tidy 

  • Follow all infection prevention and control processes 

  • Support the program volunteers in their role 

  • Collect and record program fees and maintain accurate records of the group/activity attendance as required 

  • Report incidents/accidents following correct procedures and record daily events/incidents in communication diary 

  • Answer general telephone enquiries, record relevant details and action as required 

  • As required, plan, purchase resources, prepare, cook and present an innovative menu with reference to client's dietary requirements 

  • Implement Communities@Work's Food Safety Program in daily routines and support Activity Facilitators and Volunteers to adhere to program requirements 

  • As required, undertake responsibility for end of day kitchen activities and procedures in accordance with the Food Safety Program, and with regard to Lifestyle Services policies, procedures and Operational Guidelines 

  • Actively demonstrate workplace behaviours consistent with the Communities@Work Purpose, Vision and Values and comply with Communities@Work policies, plans and guidelines 

  • Actively demonstrate and create the expectation that staff and volunteers will positively contribute to a consumer-directed and person-centred approach and reflect the Communities@Work values to inform service delivery 

  • Treat all participants, their families and other stakeholders with respect and be inclusive of different cultures, religions and beliefs 

  • Engage actively with the Communities@Work Performance Development Framework (PDF) and participate in identified professional development activities 

  • Credibly represent Communities@Work and Lifestyle Services in a professional manner when in public, at relevant forums or when attending training 

 

Skills and Experience 

Essential 

  • Demonstrated ability to contribute to the development of inclusive and innovative activities and programs within a Wellness and Reablement framework 

  • Experience in planning, delivering and evaluating group activities that could include, but not be limited to, art and craft, nutrition and cooking, life skills, fitness, technology or academic topics to seniors 

  • Ability to work with aged persons or persons with disabilities using a best practice approach that is compliant with the legislative requirements and consistent with the National Standards for Disability and the Aged Care Quality Standards 

  • Ability to understand the needs of senior members of the community  

  • Demonstrated sound communication and organisational skills 

  • Attention to detail and the ability to independently time manage tasks. 

  • Current driver's licence and access to own vehicle  

  • Maintain current 'Working with Vulnerable People' registration 


Desirable
 

  • Certificate III in aged/community care or equivalent is desirable. 

  • Experience supporting people living with dementia 

  • Qualifications and/or experience in catering for groups 

  • Current First Aid and Food Handling Certificate or willingness to achieve
