In-Home Care and Community Support

We offer a range of basic support services under the Commonwealth Home Support Programme (CHSP) to assist older people to live in their own homes for as long as possible, to maximise their independence and access to the community.  These in-home and community services for seniors include the following supports:

1. Domestic assistance:

for example, we can organise an ongoing basic clean of your home and provide assistance to complete tasks that you may be finding difficult to do by yourself (tasks such as vacuuming and mopping floors, bathroom and kitchen cleaning, changing bedding etc).

2. Social support (Individual):

for example, we can provide a support worker to take you shopping or to accompany you to other regular appointments where you may need some assistance.

3. Transport:

you can use our transport services to attend appointments, go shopping, attend our seniors’ social groups, etc. Visit our Transport page for more information (click here).

How to access In-Home Care and Community Support Services for Seniors

Please note that these services are subject to availability and a registration process.

1. Registration with My Aged Care

You must be registered with My Aged Care and have a referral for the service type that you need, such as Domestic Assistance, Social Support (Individual), Transport or Social Support (Group). If you are not sure if you are registered with My Aged Care, or if you are registered but are not sure if you have the referral you need, you can contact My Aged Care on 1800 200 422.

2. Service Confirmation with Communities at Work

When you contact us with your referral details, we will confirm your eligibility and advise on the availability of the service you require. Following confirmation of availability, we will complete a registration process with you either by phone or by visiting you in your home.

3. Ongoing Service Usage 

Once services are in place, we will check in with you from time to time to make sure everything is going as it should and determine whether any changes need to be made.
