NDIS Support Coordination Services
In addition to our Seniors, Transport, and Disability Services under our Lifestyles program, Communities@Work offers NDIS Support Coordination Services, as we are a registered NDIS Provider. To be eligible for this service you must have a current NDIS plan that includes Supports Coordination.
Support Coordination is designed to help support you to navigate and understand more about the services and supports that are available to you via your NDIS plan along with other programs and mainstream supports.
Our team will help you navigate and implement your NDIS plan, provide you with information about service providers who can help you make the most of your NDIS funding, and work towards resolving challenges with your services. We aim to support you to exercise choice and control, learn how to implement your NDIS plan yourself, strengthen your informal support network, and explore community and alternative support options.
We adopt a practical and person-centred approach to your support and encourage you to achieve your goals and manage your support needs. With a large network across Canberra, our experienced NDIS Support Coordination team can provide you with links to suitable services, groups and opportunities.

Community Assistance Support Program Coordination Services
As a long-standing provider of disability and broader community services, we can offer you Assessment and Care Coordination through the Community Assistance Support Program (CASP), if you are not eligible for NDIS and are experiencing health conditions that are impacting on your independence. This program provides a short term, goal-focused service.
To access CASP services, you should be living in the ACT, be aged under 65 years and require home and community support for daily living activities due to a health-related issue such as:
• An illness that comes and goes
• Short-term health or mental health conditions
• A need for post-hospital care and support, or assistance with hospital outpatient visits
• A disability that is not of a ‘major and permanent’ nature (as required for National Disability Insurance Scheme (NDIS) eligibility
• Any combination of any number of these characteristics
• Are the unpaid carers and family members of any of the above individuals
Our coordination services can support you with:
- Linking to services that will support you to remain independent in your own home
- Accessing health services for better health outcomes
- Information & support to apply for the NDIS
- Working towards realisation of short term health-related goals
